Traditionally, these were managed care plans providing a rich level of benefits emphasizing preventative care through an exclusive network of providers and significant cost controls with the option to receive a moderate level of benefits out of network. They were similar to an HMO plan except that they would allow some non-network coverage. More recently, the POS plan has become very much like a PPO with essentially no defining differences.
